- The SAFER Matrix: A New Scoring Methodology Perspectives, May 2016, Vol 35, #9, Pg 1– Along with the deletion of 225 elements of performance, this new approach to scoring, catagorizing and prioritizing standards is the hottest new thing from TJC”s Project Refresh (See article #2 below). That is especially the case for our readers because this new approach will first be applied to all Psychiatric Deemed Hospital Surveys starting next month (June 6th). The new matrix will be “embedded” into the survey process and survey reports. The good news is that SAFER eliminates a slew of sometimes confusing requirement labels (e.g., direct, indirect, category A/C, Risk). These are being replaced with a reliance on surveyor judgement regarding the scope and severity of safety risk in each finding. TJC reports high inter-rater reliability even before surveyors have received intensive training, but they will be very interested in receiving feedback from the field.
