The ten most frequently reported types of sentinel events have not changed much since the first half of last year.  Fire-related events dropped off and Criminal Events returned as #8.  Suicide fell to #4 while Falls moved up to # 3.  Similarly, the first five of the ten most frequently identified root causes remain the same as last year except that Performance left the list and Operative Care returned.
Tip: Leaders should consider items form the list of most frequent sentinel events (e.g., falls/#3, suicide/#4, Treatment delay/#5 Med Errors/#10) as potential candidates for proactive risk assessment that might help your organization prevent/avoid such incidents
