- Targeted Solutions: Joint Commission Center for Transforming Healthcare Takes on Patient FallsThe Source, September 2015, Vol 13, #9, Pg 7 J– The U.S. Department of Health & Human Services estimates that 700,000 to 1 million people fall in US hospitals each year. According to this article 30%-35% of those falling will suffer injuries and approximately 11,000 of them will die from those falls. The article describes TJC’s newest (their 4th) Targeted Solutions Toolkit (TST) which focuses on preventing falls. The customizable solutions in this TST have been proven to reduce overall hospital rates of falling by 25% and the rate of falls with injuries by 50%.
