This 435-bed, not-for-profit general and acute care facility credits a 25% mortality rate reduction to its multi-year efforts to become a high reliability organization.  The article outlines key steps and strategies in the approach they took. This included strong leadership, medical staff commitment, a revamping of their PI process and special training. The truing support was a three-pronged approach that focused on Just Culture, Team building and the Crucial Conversations method for improving communication effectiveness. The 5-page CaroMont Health Performance Improvement Plan (CHPIP) for 2014 is also attached.
Tip: LDR and PI should at least review the Goals & Objectives section of the CHPIP and then consider next steps toward high reliability for your organization.
