This installment of the Tracer 101 series plays off of standard CTS.02.01.05 that requires Community Mental Health Centers to have written policy for, performing and documenting a physical screening.  The lack of a documented physical screen is the trigger for this tracer scenario and it includes 5 sample tracer questions that are nicely integrated into the tracer narrative.
Tip: Spot check a sample of 10 CMHC records to determine compliance with CTS.02.01.05.  If findings reveal significant non-compliance, consider including this topic in your mock survey tracers.
