• TS: – Perspectives on Patient Safety™: New Paper from The Joint Commission Encourages Collaboration in Transitions of Care[®] [REF: JCSC, LDR, SW] The Source, April 2013, Vol 11, #4, Pg 15

The Joint Commissions Transitions of Care Portal is continuing to evolve.  It has now released its 2nd in a planned series of papers addressing the challenge of successful transition/discharge.  This one is entitled, “The need for collaboration across entire care continuum” (8-page PDF).  It identifies 7 fundamentals needed to ensure safe transitions.  They include:

1. Leadership support
2. Multidisciplinary collaboration
3. Early identification of patients/individuals at risk
4. Transitional planning
5. Medication management
6. Patient and family action/engagement
7. Transfer of information

In addition, this article also provides early warning of “potential new standards and an accompanying survey process, to help Joint Commission–accredited organizations provide safe, high-quality transitions of care…”.
Tip: JCSC and LDR should take heed to the early warning and review the new article referenced here and its forerunner, “The need for a more effective approach to continuing patient care”.  It is only 8 pages of large-print monograph reading 🙂
