Evaluating the effectiveness of your medication management system (MMS) is required by MM.08.01.01.  Failure to do so in an organized manner is one of the most common reasons for citations related to this standard. [Pearl]  The real take home in this article, however, is to understand the emphasis this standard now places on a deliberate and formalized performance improvement approach (e.g., PDCA, FADE) to evaluation of the medication management process.  This includes collecting baseline data on your process, then analyzing and comparing that data over time.  It means identifying opportunities to improve, implementing actions to do so and evaluating the effectiveness of those efforts against measurable performance indicators.  
Tip4:  Perform (or update) an FMEA on your entire medication management system and based on the results, periodically (i.e., every 2-3  years) conduct more focused FMEAs on some aspect/phase of your MMS (e.g., medication reconciliation).
