•PR: The Joint Commission Launches Transitions of Care Portal Resources to Improve Safety in Patient Care Transitions [REF:LDR, PI UR/UM] Perspectives, December 2012, Vol 32, #12, Pg 1 [®] As we indicated last month (see RTNrvw: Tracer Methodology 101: Transition of Care Tracer) Transitions of Care (TOC) appears to be an issue of emerging importance for Joint Commission Enterprises (i.e., The Joint Commission, Joint Commission Center for Transforming Healthcare, and Joint Commission Resources). “Transitions of care” refer to the movement of patients between health care practitioners, settings, and home as their condition and care needs change.” Think effective discharge planning/continuity of care/hand-off communication. This article provides a 1-page overview of the TOC Portal to include how to access and use it. It also briefly describes the type of information available from each member of JC Enterprises including the recent 8-page article on TOC. This portal is to be updated quarterly and is projected to be just the first in a series dedicated to this topic. Also be aware that a number of other organizations such as the Agency for Healthcare Research and Quality (AHRQ) and National Transitions of Care Coalition (NTOCC) have also been working on this issue since at least 2008.
