*TS: Benchmark: Improving Performance on Key Measures Related to Transitions of Care[®] [REF: JCSC, PI, SW] The Source, September 2014, Vol 12, #9, Pg 16 JCs1409_B3 The performance rate on accountability measures improved by almost 16% between 2002 and 2012. However, the rate of 86.7% for HBIPS-7 (Post discharge continuing care plan transmitted) was significantly below the composite performance rate of 97.6% for all measures. This article provides 4 strategies for improving HBIPS performance. The first and rate limiting step is to actually identify a care provider at the next level. Next, the approach to transmitting the discharge/after care plan should be standardized to ensure it includes all the required elements; especially medications, dosages and indications for their use. Finally, performance may be enhanced through education of relevant facility staff AND key outside organizations (e.g., next level providers) about the importance/need for timely (with 5 days) communication of discharge plans. |
